What are Therapy Services?

When a patient is using an automated peritoneal dialysis (APD) cycler powered by Vantive CONNECT Clinician (Sharesource Connectivity Platform), Vantive's Therapy Services (PD Telecare) team provides proactive technical support to the patient and actionable insights to their clinician. The Therapy Services team remotely monitors the patient's treatment data during their entire therapy journey.

How do Therapy Services work?

A dedicated Vantive Therapy Services nurse reviews Clinical Portal (Sharesource Clinical Portal) alert and alarm trends from Amia and Homechoice Claria automated peritoneal dialysis (APD) cyclers to identify whether an enrolled patient needs additional technique or training support. If support is required, a Vantive technical support representative will call the patient.

In parallel, a Therapy Services nurse notifies the patient’s PD clinic nurse of potential clinical issues, and the PD clinic nurse decides on appropriate clinical intervention. The clinic nurse also receives a monthly report summarizing all PD Telecare interactions with their patients.

The Clinical Portal is a globally available two-way digital platform allowing clinicians to remotely monitor and manage patients from the clinic or anywhere there is access to the internet, including the ability to change a patient’s prescription setting.

The Sharesource Connectivity Platform portal is intended for use by healthcare professionals to remotely communicate new or modified treatment parameters with compatible dialysis instruments and transfer completed treatment data to a central database to aid in the review, analysis, and evaluation of patients’ historical treatment results. This system is not intended to be a substitute for good clinical management practices, nor does its operation create decisions or treatment pathways.

MyPD Patient Application is intended for use by dialysis patients to receive new or modified treatment parameters from a healthcare professional, to view the collection of treatment data that can be remotely communicated to Sharesource Connectivity Platform and to view their historical treatment results. It is intended for use with commercially available mobile platforms and uses generally available networks and communication protocols. 

MyPD Patient Application is not intended to control or perform the treatment of patients undergoing dialysis. This system is not intended to be a substitute for good clinical management practices nor does its operation create decisions or treatment pathways.
